Referring a Patient
EPIC USERS
Do you utilize EPIC as your EHR/EMR? Referring patients through Epic can streamline the process and ensure accurate communication of patient information.

If you are a healthcare provider within Hartford Healthcare or Starling Physicians, we encourage you to submit patient referrals through EPIC to department CC COLON RECTAL PLNVL or follow the referral process below:

1. Type in Amb Referral to Colorectal Surgery under the “Place Amb Orders Tab”.
2. Keep class as Internal Referral.
3. Under the referral department select CC COLON RECTAL PLNVL.
4. Under Provider, if you prefer your patient to see a specific physician, please select Dr. Christina Czyrko or Dr. Maria Christina Mirth.
5. Under Referral Type please select Colonoscopy or Consult.

If there are additional details or specific instructions that you would like to include, please make sure to provide them in the comment section to ensure a smooth referral process.
NON-EPIC USERS
We understand that everyone has unique needs and how important it is to have access to care. That’s why we have made it easy for providers who do not use EPIC to refer patients to our office by providing a secure fax line. All you or your staff need to do is manually fax a referral with the most recent office note, medication list, and EKG (if available) to (860) 826-3883. This ensures that all our patients receive the same level of care, regardless of their provider.
At Connecticut Colon & Rectal Surgery, LLC, we are dedicated to providing exceptional care and services to our patients. We understand that time is of the essence, therefore, our office is committed to providing timely, efficient and reliable service. If appropriate and availability allows, a consultation appointment will be scheduled for your patient to be seen by one of our experienced physicians, generally, within a two-week timeframe of contacting the referred patient. Procedures are generally scheduled shortly thereafter.
